There are two (2) ways to enter:
- Survey Completion. Recipients of a Survey Promotion Invitation will automatically receive one entry into the Promotion by completing the survey in its entirety and successfully submitting same so it is received on or before the entry deadline. Entrants must complete all required information on the survey form to be eligible for the Sweepstakes.
- By Mail: To enter this Sweepstakes without completing the survey, on a plain piece of 3”x5” paper, hand-print your complete name, address (including city, state and zip code), age, email address, daytime telephone number and the name of this Sweepstakes [Lee Health Panel Drawing]. Mail your entry in a #10 business-size envelope with first-class postage affixed, to: Market Street Research Survey Sweepstakes, 9 ½ Market Street, Northampton, MA 01060. Mail-in entries must be postmarked by the entry deadline and received no later than five (5) days thereafter.
- Winners will be selected in a random drawing from among all eligible entries received. The odds of winning will depend upon the number of entries received.
- Prizes – $100 Cash or Gift Card-
- Prize fulfillment – Once a winner is selected, the prize can be redeemed in the form a check, visa gift card, or gift card from various stores and shops.
